- Clients may not receive more than 1 time per 30 day period. For example, a client who picks up on the 1st of the month is eligible again no sooner than the 1st of the following month.
- Surplus food items (items vary daily) are available no more than once per week to clients without needing to obtain a referral form from an approved referring agency. These items can be utilized in addition to regular monthly food orders.
- Picture Identification must be provided each time a client picks up a food order.
- Clients must reside in La Plata County and provide acceptable documentation showing residence if their Picture Identification does not.
- Any client not physically able may send a copy of their picture ID as well as a written authorization in order to allow food to be picked up on their behalf.
- ALL MEMBERS LIVING AT THE SAME ADDRESS MUST BE LISTED ON THE SAME FORM. Clients found to be receiving separate orders for the same residence may be placed on a temporary or permanent ban.
- Clients must obtain a new Referral Form from an approved referring agency each month they intend on receiving food, regardless of the number of previous times they have accessed the Food Bank for assistance.
- All Referral Forms must be FAXED to the Food Bank by the Referring Agency. Incomplete &/or non-faxed Referral Forms will not be accepted. Special arrangements may be made with a Food Bank representative if an exception is needed.
- Any clients residing at the Safe-house or Community Shelter are not eligible to receive Food Bank Assistance, as foods to prepare all meals are provided to their residents.
- We will not tolerate abusive, dishonest or inappropriate behavior! Clients demonstrating these behaviors will face disciplinary action, including a potential permanent ban from receiving services.
- Prior approval must be obtained from Food Bank representative in order to accept a non-signed (ie called in request) order from a Referring Agency.
