Identification and Residency Verification Requirements

Valid Forms of Picture Identification (ID)

  • Driver’s License or State Identification Card (issued within the United States and not expired)
  • College/University Issued Student ID
  • Passport
  • Military Identification Card
  • Immigration Card (resident alien)
  • Consulate Identification Card (As Proof of Picture ID ONLY – NOT RESIDENCY)

Proof of Residency (If Not Listed on Primary Picture ID):

Must display the applicant’s name and principal La Plata County residence address (NO PO BOX), which may include:

  • Bank Statement (dated within 30 days);
  • Mortgage or Rental/Lease Contract (Not Handwritten, Current);
  • Major Credit Card Bill (dated within 30 days);
  • Property Tax Notice (statement or receipt dated within one year);
  • School Transcript (dated within 90 days);
  • Utility Bill (no cell phone bills will be accepted);
  • Valid Colorado Vehicle Registration or Title;
  • Wage Statement (showing physical address)
