- Consulate Identification Card (As Proof of Picture ID ONLY – NOT RESIDENCY)
Proof of Residency (If Not Listed on Primary Picture ID):
Must display the applicant’s name and principal La Plata County residence address (NO PO BOX), which may include:
- Bank Statement (dated within 30 days);
- Mortgage or Rental/Lease Contract (Not Handwritten, Current);
- Major Credit Card Bill (dated within 30 days);
- Property Tax Notice (statement or receipt dated within one year);
- School Transcript (dated within 90 days);
- Utility Bill (no cell phone bills will be accepted);
- Valid Colorado Vehicle Registration or Title;
- Wage Statement (showing physical address)
